Welcome New Clients!  Please download the following paperwork. Because my email and this webpage are not secure and encrypted, it is advisable that you print out these forms and either mail them to my office (5182 Katella #206, Los Alamitos, CA 90720) or bring them with you to your first appointment.

The first page is basic information: your name, address, date of birth, etc. Just a quick note about the indication for “gender”: this is something that we need only for insurance purposes. So, when filling out this space please indicate the gender that you indicated to your insurance company. If you’re not using insurance, feel free to leave the space blank! I feel very strongly about inclusivity and would gladly omit this particular question if the choice was mine to make.

The next four pages require your careful reading, initials and signature. These detail the Office Policies, General Information, the Agreement to provide Mental Health Services, the Fee Schedule and Fee Agreement, and important information about my cancellation policy. I recommend that you carefully read through all of these pages and if you have any questions, I will be happy to discuss and clarify anything for you at our first session. If you print out these forms and bring them with you to your first session, it saves us both a little time but I always have copies of these forms available in the office.
